The groan is the prayer of the body before the mind has language. Scripture records groans at the deepest levels of suffering, longing, and hope. Israel groaned under Egyptian slavery and God heard their groaning (Ex 2:23-25). Creation groans as in the pains of childbirth awaiting redemption (Rom 8:22). Believers groan within themselves, waiting for adoption and the redemption of the body (Rom 8:23). Even the Spirit groans with inexpressible groans in intercession (Rom 8:26). Groaning is not unbelief; it is hope without vocabulary yet.

GROAN, v.i. [Sax. granian.] To utter a mournful voice, as in pain or sorrow; to sigh deeply with the breath audible and prolonged. In Scripture, groaning is the voice of the body in distress, the prayer of those without words, the sound of creation waiting for redemption, and the intercession of the Holy Spirit on behalf of the saints — groans too deep for articulation which God hears as prayer.

Modern Christians often feel they must pray in polished sentences. Scripture legitimizes the groan — a prayer God hears perfectly even without words.
The tidy Christianity of polished prayers can leave suffering people voiceless. Scripture legitimizes the groan: Israel groaned and God heard it as prayer. The Spirit groans within us when we cannot find language. This is enormous pastoral permission: your inarticulate grief, your wordless longing, your midnight tears — these are prayer, fully received by the Father, fully interceded by the Spirit. Do not feel pressure to craft eloquence when you are in the ash heap. Groan. God understands the sound.
